Home Health Physical Therapist Assistant

Administer physical therapy treatment plans to patients in their homes, including non-complex manual exercises, therapeutic massage, and various modalities like traction, heat, cold, and electrical treatments. Document client records and submit clinical documentation weekly while applying infection control and universal precautions. Notify the Physical Therapist or Branch Director of equipment failures, supply shortages, or staff/client changes. Attend educational meetings and coordinate department in-services. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization to regain strength, mobility, and independence.
