Job Summary
Administer physical therapy treatment plans to patients in their homes, including non-complex active and passive manual therapeutic exercises, massage, traction, heat, cold, water, and electrical modalities. Document client records and submit clinical documentation to the office weekly. Coordinate department in-services and notify the Physical Therapist or Branch Director of equipment failures, supply shortages, or staff/client changes. Apply infection control and universal precautions during all patient care activities. This role supports CenterWell Home Health's mission to help patients regain strength, mobility, and independence from the comfort of their homes.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
- TB screening
- Maintain personal vehicle liability insurance in accordance with state minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
