Job Summary
Deliver personalized in-home physical therapy as part of CenterWell Home Health, administering the therapy plan in patients' homes, performing active/passive manual therapies, therapeutic massage, traction, heat, cold, water, and electrical modalities, documenting records weekly, coordinating department in-services, and ensuring infection-control practices; requires state PT license, driving eligibility, CPR, and teamwork within a geriatric care context; pay range $70,400–$96,300 per year for full-time at 40 hours/week.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver's license, auto insurance and reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
