Job Summary
Administer the physical therapy treatment plan to patients in their homes, including non-complex active and passive manual therapeutic exercises, massage, traction, and various modalities. Document client records and submit clinical documentation to the office weekly. Coordinate department in-services, notify supervisors of equipment failures or supply shortages, and apply infection control and universal precautions during patient care. This role supports patients managing chronic conditions or recovering from injury, illness, or surgery through personalized home care delivered by a multidisciplinary team.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
- TB screening
