Job Summary
The Home Health Physical Therapist Assistant will perform direct physical therapy services under the supervision of a Physical Therapist, following the established Plan of Care and physician's orders. This role requires adherence to accepted standards of practice and state guidelines while ensuring quality and continuity of care. Key responsibilities include verifying the Plan of Care before each visit, incorporating patient care goals into therapy, and reporting any changes in patient status back to the Physical Therapist.
Required Qualifications
- Graduate of an accredited Physical Therapy Assistant program approved by the American Therapy Association
- Currently Licensed as a Physical Therapy Assistant
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable transportation
