Job Summary
Conduct physical therapy services in patient homes to help clients regain independence and navigate their living environments. Collaborate with interdisciplinary teams to deliver patient-centered care while maintaining flexibility and a healthy work-life balance. Provide clinical mentorship and support within a supportive environment that values expertise and dedication.
Required Qualifications
- Current license as a Physical Therapist or ability to obtain in the state of practice
- Minimum one (1) year experience in a health care setting
- Must meet Clinical Competency requirements
- Ability to travel to patient homes within designated geographic area
- Valid driver's license and proof of auto insurance
- Current CPR certification
