Home Health Physical Therapist

Provide leadership to the therapy team while ensuring compliance with company policies, CHAP, and federal regulatory requirements. Implement and maintain quality assurance programs and coordinate closely with the Medical Team to ensure high client satisfaction. Conduct therapy and supervisory visits for PTAs as availability permits, devising client-centered treatment plans based on individual assessments. Maintain current working knowledge of Medicare Conditions of Participation. Requires a Michigan-licensed Physical Therapist with at least one year of home care experience.
