Job Summary
Provides in-home physical therapy services in the St. Cloud, MN area under the Plan of Care. Administers evaluations, develops the Plan of Care, and delivers PT treatments and activities to clients in their homes. Supervises PTAs and HHAs as appropriate; maintains documentation and progress notes; communicates with physicians, Case Managers, families, and staff. Travels to clients’ homes for 1:1 sessions (1-3 hours per client) and supports functional activities of daily living, equipment/assistive devices, and home modifications where needed. Maintains confidentiality, participates in case conferences, andEducates families on home physical therapy programs, patient progress, and strategies to adapt to physical, mental, and social limitations. Ensures a safe home environment and adherence to policies, with a focus on person-centered, individualized care.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Currently licensed as a Physical Therapist in Minnesota with no revocation/suspension/limitations
- At least two (2) years of experience in a health care setting; home care experience preferred
- Strong interpersonal communication and teaching skills
- Strong organizational skills and ability to work independently
- Excellent written and oral communication skills
- Access to dependable vehicle or public transportation for travel between sites
- Valid driver’s license and proof of car insurance when using a personal vehicle for company business
- Proof of negative mantoux or chest x-ray documentation
- CPR certification highly recommended
- U.S. Citizenship or evidence of valid Alien Work Permit
- Ability to pass background checks including Minnesota Dept. of Health and OIG Excluded Entities
