Job Summary
Provides in-home physical therapy services to clients under the Plan of Care, conducting PT evaluations, developing and implementing treatment plans, and guiding families in home exercise programs. Part-time role with a flexible schedule Monday–Friday; travel to clients’ homes for 1:1 sessions (1–3 hours per client) and coverage for multiple locations in the Mankato, MN area including Albert Lea, Alden, Ellendale, and Blue Earth. Reports to Clinical Supervisor; opportunities for productivity bonuses and mileage reimbursement; requires supervision of PTAs and HHAs as appropriate; emphasizes patient education, documentation, and coordination with physicians, case managers, and families. Includes maintenance of records, safety, confidentiality, and participation in team activities and community resources.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Minnesota license as a Physical Therapist (not revoked or restricted)
- At least two (2) years of experience in a health care setting (home health care preferred)
- Strong interpersonal and teaching skills
- Ability to work independently
- Excellent written and oral communication skills
- Access to dependable vehicle or public transportation for travel to client homes
- Valid driver’s license and proof of car insurance for business use
- Proof of negative mantoux or negative chest x-ray
- Current CPR certification (highly recommended)
- U.S. Citizenship or evidence of valid Alien Work Permit
- Willingness to undergo background checks including Minnesota Department of Health and OIG List of Excluded Individuals and Entities
- Ability to maintain confidentiality and safe client environment
