Job Summary
The Home Health Physical Therapist provides in-home, one-on-one physical therapy services to clients in Little Falls and surrounding areas (coverage from St. Cloud, MN office). Responsibilities include conducting PT evaluations, developing and implementing the Plan of Care, delivering treatments and therapeutic procedures, training families and staff, supervising PTAs and HHAs, documenting progress, coordinating with physicians and Case Managers, and ensuring patient safety and confidentiality. The role requires travel to clients’ homes, flexible full-time or part-time scheduling, and collaboration within a team to support client independence and quality of life.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Minnesota PT license without limitations or restrictions
- At least two (2) years experience in a health care setting (home care preferred)
- Valid driver’s license and reliable vehicle or access to transportation
- CPR recommended/required
- U.S. Citizenship or evidence of valid Alien Work Permit
- Background checks and screenings including Minnesota Department of Health and OIG list
- Ability to travel to multiple company locations
- Strong interpersonal communication and teaching skills
- Excellent written and oral communication
- Ability to work independently and manage schedule
- Ability to obtain negative Mantoux or chest x-ray proof
