Home Health Physical Therapist (58333)

Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans as needed while assisting with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on their plan of care and coach families on safely assisting with exercises. Work as part of a home health team focused on the patient's plan of care and goals. Requires a graduate of an accredited Physical Therapist Program with an active Minnesota license, minimum one year of experience, and CPR certification. Salary ranges from $90,000 to $105,000 with bonus opportunities and mileage reimbursement.
