Job Summary
Deliver skilled, patient-centered care to home health patients in the Justice area, performing comprehensive evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. Educate patients and caregivers on therapeutic exercises and injury prevention, while working independently within an assigned territory and ensuring thorough EMR documentation. Travel to patient locations, spending approximately 20% of your day driving, and collaborate with an interdisciplinary team to solve problems in real time. This role includes a $20,000 total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on bonus, for a Physical Therapist with one year of experience and a current state license.
Required Qualifications
- Active Physical Therapist, PT license in the state you work
- One year of experience as a Physical Therapist
- clinical expertise to deliver quality patient care
- Current driver's license
- ability to spend ~20% of your day driving to/from patient locations
