Job Summary
Provide one-on-one rehabilitation in patients’ homes to help regain strength, restore mobility, and return to daily activities. Assess functional abilities, develop individualized treatment plans, and guide interventions to improve mobility, balance, strength, and safety. Work with an interdisciplinary care team, educate patients and caregivers, and travel daily within the Monroe County area to patients’ homes, assisted living facilities, or nursing homes. Position involves a high degree of autonomy in planning and delivering home-based PT services, with a focus on reducing pain and improving independence. Requires a licensed Physical Therapist with a Master’s or doctoral degree, current Michigan license, and willingness to travel.
Required Qualifications
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
