Job Summary
Assess patients' functional abilities and home environments, then develop individualized treatment plans to restore mobility, balance, strength, and safety. Provide one-on-one rehabilitation care in patients' homes, assisted living facilities, or nursing homes to help individuals recover from surgery, injury, or illness while reducing pain and supporting greater independence. Guide patients and caregivers through targeted interventions that improve function and facilitate a return to meaningful daily activities through measurable progress toward their goals.
Required Qualifications
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation
- Valid driver's license
- Valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Desired Qualifications
- Home health experience
- OASIS certification or willingness to obtain
- Experience with Medicare home health documentation
- Strong clinical assessment and treatment planning abilities
- Knowledge of medical terminology and home health regulations
- Excellent verbal and written communication skills
