Job Summary
Assess patients' functional abilities and home environments, then develop individualized treatment plans to restore mobility, balance, strength, and safety. Guide patients through targeted interventions for recovery from surgery, injury, or illness while educating caregivers and supporting independence. Collaborate with interdisciplinary teams to ensure measurable progress toward quality-of-life goals. This high-travel position requires daily visits to patients in homes, assisted living facilities, or nursing homes within Livingston County, with a Monday-to-Friday schedule from 8 a.m. to 5 p.m.
Required Qualifications
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Desired Qualifications
- Home health experience
- OASIS certification or willingness to obtain
- Experience with Medicare home health documentation
