Job Summary
Provide one-on-one rehabilitation care in patients' homes, assisting individuals with regaining strength, restoring mobility, and returning to daily activities following surgery, injury, or illness. Assess each patient's functional abilities and home environment to develop individualized treatment plans that improve mobility, balance, strength, and safety. Guide patients and caregivers through targeted interventions while working closely with an interdisciplinary team to educate and empower them toward measurable progress. This high travel position requires daily travel within Washtenaw County and surrounding areas to visit homes, assisted living facilities, or nursing homes. Monday to Friday, 8 a.m. to 5 p.m. schedule.
Required Qualifications
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Desired Qualifications
- Home health experience
- OASIS certification or willingness to obtain
- Experience with Medicare home health documentation
