Home Health PDN - (LPN or RN) - Private Duty Nurse [PRN or Part-Time > Option for Full-time]

Job Summary

Required Qualifications

• Active LPN or RN license in good standing in the State of Ohio
• Minimum of one (1) year of experience in a home healthcare setting, acute care setting, or equivalent nursing experience
• Successful completion of HCN's written skills test and/or competency evaluation
• Meet/completion of all training certifications required by the state
• CPR and First Aid Certifications (active/current preferred)
• Ability to perform main physical tasks regularly and safely
• Strong background in nursing, patient care, safety, compliance, and confidentiality
• Good interpersonal skills and compassion
• Demonstrated ability to read, write, and follow a Plan of Care
• Reliable, honest, punctual, articulate, compassionate, detail-oriented, self-driven, problem-solver, compliant, and confidential
• At least eighteen (18) years of age
• Valid driver's license/ID and reliable transportation to/from client locations.
• Consent and ability to pass a State and/or Federal Background Check.

Desired Qualifications

• 3-5 years of paid experience in a home health or caregiving setting
• Paid experience with a Home Health Agency
• Specialized experience in: G-tube care, trach care, vent or Trilogy care, infants, persons with disabilities, working with family caregivers, various disease states or medical conditions
• EMR/EHR experience and proficiency (ContinuLink highly desirable)
• Willingness to promote Home Care Network throughout the community and social networks
