Job Summary
Provide occupational therapy in a home-health setting to help patients regain strength, mobility, and independence. Collaborate with physicians and the clinical team to develop, implement, and revise therapy plans addressing daily living activities, work-related skills, and community reintegration; supervise and coordinate with OTAs and home health aides; train staff and families on retraining and activities of daily living; ensure comprehensive documentation including evaluation, treatment plans, progress reports, and discharge summaries; participate in care coordination and discharge planning; contribute to team and family education and support for patient independence.
Required Qualifications
- Current and unrestricted Occupational Therapy license in the state of North Carolina
- A valid driver‘s license, auto insurance, and reliable transportation
- Current CPR certification
