Job Summary
Evaluate patients and develop individualized treatment plans to improve independence and quality of life. Provide occupational therapy services according to the established Plan of Care, assess progress, and modify plans as needed. Document evaluations, treatments, and outcomes accurately while educating patients and caregivers on adaptive equipment and home safety. Collaborate with physicians and the interdisciplinary care team, ensuring compliance with all state, federal, Medicare, and agency regulations. Requires an active Florida OT license, minimum one year of home health experience, and valid driver's license. Flexible scheduling with per-visit compensation.
Required Qualifications
- Active Florida license as an Occupational Therapist (OT)
- Ability to work independently and manage a flexible schedule
- Reliable transportation and a valid driver's license
Desired Qualifications
- Minimum of 1 year of home health experience preferred
- Experience with electronic medical records (EMR)
- Knowledge of Medicare guidelines and home health documentation standards
- Bilingual (English/Spanish) is a plus
