Home Health OT

Evaluate patients and develop individualized treatment plans, then provide occupational therapy services according to the established Plan of Care. Assess patient progress to modify treatment plans as needed, while documenting evaluations, treatments, and outcomes accurately. Educate patients and caregivers on adaptive equipment, home safety, and therapeutic exercises, and collaborate with physicians and the interdisciplinary care team. Maintain compliance with all state, federal, Medicare, and agency regulations. Requires an active Florida OT license, minimum one year of home health experience, and ability to manage a flexible schedule.
