Job Summary
Evaluate patients, develop individualized treatment plans, and provide occupational therapy services according to the established Plan of Care. Assess patient progress, modify treatment plans as needed, and document evaluations, treatments, and outcomes accurately. Educate patients and caregivers on adaptive equipment, home safety, and therapeutic exercises while collaborating with physicians and the interdisciplinary care team. Maintain compliance with all state, federal, Medicare, and agency regulations. This role requires an active Florida license, minimum one year of home health experience, and ability to work independently with a flexible schedule.
Required Qualifications
- Active Florida license as an Occupational Therapist (OT)
- Ability to work independently
- Manage a flexible schedule
- Reliable transportation
- Valid driver's license
Desired Qualifications
- Minimum of 1 year of home health experience
- Experience with electronic medical records (EMR)
- Knowledge of Medicare guidelines and home health documentation standards
- Bilingual (English/Spanish)
