Confer with physicians and clinical teams to develop, implement, and revise therapy treatment programs focused on daily living, work-related skills, and adaptive equipment needs. Participate in care coordination and discharge planning while training staff and family members on retraining programs for dressing, feeding, and hygiene. Submit evaluation reports, treatment plans, and discharge summaries to supervisors, and provide written instructions and care plans to Occupational Therapy Assistants and Home Health Aides. Oversee staff performance and ensure appropriate documentation completion.
