Job Summary
The Occupational Therapy Case Manager plans and provides skilled occupational therapy services in clients' homes per the Plan of Care. Performs assessments, establishes treatment goals, delivers OT treatments, translates exercises into daily living activities, instructs on equipment use, modifies home environments, maintains documentation, and communicates with physicians and the Clinical Manager. Leads and mentors COTAs, HHAs, LPNs and RNs when appropriate; participates in case conferences; supports families to engage in home therapy; develops community resources; ensures confidentiality and safety; travels to client homes (1-3 hours per client) within the Mankato, MN coverage area; full-time Monday–Friday with office-hour scheduling flexibility. Maintains required licenses, completes background checks, and adheres to company policies; salary range $70,000-$100,000 with productivity/benefits.
Required Qualifications
- Currently licensed as an Occupational Therapist in Minnesota
- Graduate of an AOTA approved school of occupational therapy
- Currently certified by the AOTA to practice occupational therapy
- Have at least two (2) years experience in a health care setting or equivalent experience (home care experience preferred)
- Certification with National Board of Occupational Therapy strongly recommended
- Strong interpersonal communication and teaching skills
- Strong organizational skills and the ability to work independently
- Excellent written and oral communication skills
- Access to a dependable vehicle or public transportation to travel to multiple company stops per day
- Valid driver’s license and proof of car insurance for personal vehicle use
- Proof of negative mantoux or chest x-ray
- Current CPR certification recommended
- U.S. Citizenship or evidence of valid Alien Work Permit
- Willingness to pass background studies and screenings including those by Minnesota Department of Health and OIG List of Excluded Individuals and Entities
- Adara Home Health base salary range $70,000 - $100,000 per year plus productivity and benefits
