Job Summary
Provide skilled occupational therapy services in patients' homes. Perform initial evaluations, reassessments, and fall risk assessments (OT). Develop and implement individualized treatment plans. Improve independence with activities of daily living (ADLs), mobility, safety, and home management tasks. Educate patients and caregivers on adaptive techniques and fall prevention strategies. Collaborate with nurses, therapists, physicians, and caregivers. Maintain accurate documentation in accordance with Medicare home health standards. Schedule: Per Diem / PRN with flexible scheduling; Location: O‘ahu, HI; Licensure required as OT or COTA; Minimum one year of clinical experience; Driver's license and reliable transportation required; Hours and caseload are built to fit patient needs.
Required Qualifications
- Current Hawaii license as an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA)
- Minimum one year of clinical experience (home health experience preferred)
- Strong communication, organizational, and documentation skills
- Ability to work independently in the field
- Valid driver’s license and reliable transportation
- Passion for serving seniors, kupuna, and local families
