Job Summary
Occupational Therapist (OT) to provide home health services across Virginia, Eveleth, Gilbert, Hibbing, and Chisholm communities. Responsible for initial and ongoing functional assessments, development and modification of the OT plan of care, delivery of therapy services per physician orders, and timely documentation to support care outcomes. Duties include discharge planning, coordination with the healthcare team, education for patients and families, and supervision of OTA/HHA staff as applicable. Requires current state OT license, degree in occupational therapy, CPR, and a commitment to high-quality patient outcomes in a home health setting. Full-time position with Monday–Friday visits and a $15,000 Sign-On Bonus; comprehensive benefits package and opportunities for ongoing professional development. Location is in Minnesota with local community-based patient visits.
