Home Health Occupational Therapist - PRN

Provide skilled occupational therapy services in accordance with physician orders and individualized plans of care. Evaluate patients, develop personalized treatment programs, and assess ability to perform activities of daily living to recommend adaptive techniques. Document evaluations, treatments, and progress accurately while collaborating with physicians and other healthcare professionals. Educate patients and caregivers on home safety and ongoing therapy goals, supervising Certified Occupational Therapy Assistants as appropriate. Work primarily in daylight hours with flexible PRN scheduling and one-on-one patient care in the comfort of patients' homes.
