Job Summary
Provide skilled in-home OT care to homebound geriatric patients in Sacramento County, performing in-home assessments for ADLs and IADLs, developing adaptive strategies and adaptive equipment plans, and coordinating visits based on preferred caseload. Collaborate with a care team to ensure a holistic plan of care and maintain compliant documentation within TherapySync EMR.
Required Qualifications
- Valid California Occupational Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
