Job Summary
Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients. Implement therapeutic interventions focusing on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence. Schedule and conduct home visits based on your availability and patient needs while collaborating with a supportive care team to ensure coordinated patient outcomes. Manage your caseload and utilize the TherapySync EMR for compliant charting, with the company handling referrals, authorizations, and DME. Valid California Physical Therapist license, CPR or BLS certification, and a driver's license are required.
Required Qualifications
- Valid California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
