Job Summary
Conduct in-home occupational therapy assessments to address activities of daily living, instrumental ADLs, and home safety. Develop personalized treatment plans focusing on energy conservation, adaptive equipment, and functional strengthening. Schedule and lead patient home visits based on preferred caseload and availability while collaborating with a supportive team for coordinated care. This role requires a valid California Occupational Therapist license, CPR or BLS certification, and a driver's license. Licensed clinicians are sought across Sacramento County, including Elk Grove, Folsom, and Citrus Heights.
Required Qualifications
- Valid California Occupational Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
- Experience in home health OT or transition from outpatient or SNF settings
