Perform in-home assessments for activities of daily living, instrumental ADLs, and home safety. Create personalized treatment plans focusing on energy conservation, adaptive equipment, and functional strengthening. Schedule and conduct patient home visits based on preferred caseload and availability. Collaborate with a supportive team to ensure a holistic approach to each patient's plan of care. Manage credentials, scheduling, and invoicing through our all-in-one clinician app. Valid California Occupational Therapist license, CPR or BLS certification, and driver's license required. Territory spans South Bay to South Central LA.
