Job Summary
Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients across North County San Diego. Implement therapeutic interventions focusing on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence. Schedule and manage home visits based on availability while collaborating with care teams to ensure coordinated patient outcomes. Utilize the integrated TherapySync EMR for compliant charting and leverage our app-based network for streamlined logistics, credentials, and invoicing. Valid California Physical Therapist license, CPR or BLS certification, and a driver's license are required.
Required Qualifications
- Valid California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
