Home Health Occupational Therapist (OT)

Deliver individualized occupational therapy services, including evaluations and treatments for functional strengthening, gait training, and fall risk reduction, in the home environment of geriatric patients. Document patient responses and progress using the integrated TherapySync EMR while managing your own caseload and visit cadence. Collaborate on coordinated care plans and maintain accurate records to ensure compliance. This role supports homebound individuals across Phoenix, Mesa, Chandler, Scottsdale, and surrounding Maricopa County communities.
