Job Summary
Conduct in-home occupational therapy assessments and develop personalized treatment plans focusing on energy conservation, adaptive equipment, and functional strengthening. Manage your own caseload by scheduling and executing patient home visits to address activities of daily living and home safety needs. Collaborate with a supportive team to ensure a holistic approach to each patient's plan of care while utilizing our single EMR system for compliant documentation. Required Qualifications
- Valid California Occupational Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
- Experience in home health OT or transition from outpatient or SNF settings
