Job Summary
Conduct in-home occupational therapy assessments for geriatric patients to address activities of daily living, instrumental ADLs, and home safety. Develop personalized treatment plans focusing on energy conservation, adaptive equipment usage, and functional strengthening. Schedule and manage patient home visits based on preferred caseload and availability while collaborating with a supportive team for coordinated care. Utilize the TherapySync EMR system for documentation and receive expert consultative support to ensure compliance with professional standards.
Required Qualifications
- Valid California Occupational Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
- Experience in home health OT or transition from outpatient or SNF settings
