Job Summary
Perform in-home assessments to address activities of daily living and home safety, then develop personalized treatment plans focusing on energy conservation and adaptive equipment. Schedule and conduct patient home visits based on your preferred caseload and availability while collaborating with a supportive team for coordinated care. Work with a single EMR system for documentation and utilize our all-in-one clinician app for scheduling and billing. This role offers unparalleled flexibility for experienced clinicians or those transitioning from outpatient or SNF settings, with coverage opportunities across Sacramento County including Elk Grove, Folsom, and Citrus Heights.
Required Qualifications
- Valid California Occupational Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
- Experience in home health OT or transition from outpatient or SNF settings
- Ability to perform in-home assessments to address activities of daily living (ADLs), instrumental ADLs, and home safety
- Ability to create personalized treatment plans focusing on energy conservation, use of adaptive equipment, and functional strengthening for daily tasks
- Ability to schedule and conduct patient home visits based on preferred caseload and availability
- Ability to work with a supportive team to ensure a holistic approach to each patient's plan of care
