Job Summary
Perform in-home assessments for activities of daily living and home safety, then develop personalized treatment plans focusing on energy conservation and adaptive equipment. Schedule and conduct patient home visits based on your preferred caseload and availability while collaborating with a team for coordinated care. This role covers a diverse territory from South Bay to South Central LA, offering geographic flexibility within the region. You must hold a valid California Occupational Therapist license, CPR or BLS certification, and a driver's license.
Required Qualifications
- Valid California Occupational Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
- Experience in home health OT or transition from outpatient or SNF settings

- Spanish proficiency
