Job Summary
Home Health Occupational Therapist (OT) provides in-home therapeutic services to help patients regain independence and improve daily living skills. Responsibilities include conducting comprehensive assessments, designing and implementing individualized therapy plans, educating patients and families, utilizing exercises and adaptive equipment, tracking progress and documenting outcomes, collaborating with physicians and healthcare teams, and maintaining records in compliance with standards. Requires valid Occupational Therapy license in the practicing state; 1 year of home health or similar experience preferred. Schedule is Full-time, Monday–Friday, 9:00 AM–5:00 PM in Glendale, AZ.
