Home Health Occupational Therapist OT Full Time Salaried or Per Visit 10K SignOn Bonus

Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and collaborate with interdisciplinary team members to optimize patient well-being. Supervise other personnel as applicable and complete OASIS assessments where permitted. Serve as a resource consultant for clinical staff and ensure documentation complies with policy to prevent denials.
