Home Health Occupational Therapist OT Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide written evaluations defining needs, progress, and expected results. Deliver occupational therapy services per physician orders while maintaining plans of care through timely documentation, goal modification, and discharge planning with patient and caretaker education. Collaborate with interdisciplinary team members, physicians, and nurses to optimize patient well-being and coordinate services. Supervise other personnel as applicable and complete OASIS assessments where permitted. Document effectively to ensure compliance with policy and prevent technical or clinical denials.
