Job Summary
Deliver home health occupational therapy services to assess mobility, coordination, strength and current level of function; provide written evaluations and plan of care; administer and interpret assessments; deliver interventions per physician orders; discharge planning and home exercise instruction; document to avoid payor denials; supervise OTA/HHA as applicable; complete OASIS assessments where allowed; maintain communication with healthcare team and family; participate in interdisciplinary collaboration and ensure compliant documentation and care planning.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR and Drivers License
