Home Health Occupational Therapist OT Full Time

Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans, and manage discharge care including educating caretakers on home exercise plans. Collaborate with interdisciplinary team members, physicians, and nurses to optimize patient well-being and coordinate services. Maintain accurate documentation to ensure compliance with physician orders and payer requirements, and supervise other personnel as applicable.
