Home Health Occupational Therapist OT Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Administer, evaluate, and interpret tests to determine goals and outcomes, then deliver occupational therapy services compliant with physician orders. Provide written evaluations defining needs, goals, and expected outcomes while maintaining plans through timely documentation and goal modification. Collaborate with interdisciplinary team members, physicians, and caretakers for discharge planning and education on home exercise plans. Supervise other personnel where applicable and complete OASIS assessments. Ensure documentation meets payer requirements to avoid denials and maintain effective communication with staff and families.
