Home Health Occupational Therapist OT Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then deliver occupational therapy services compliant with physician orders. Provide written evaluations and plans of care defining needs, goals, progress, and expected outcomes while maintaining documentation to prevent technical or clinical denials. Collaborate with interdisciplinary team members, physicians, and families to optimize patient well-being, modify treatment as needed, and coordinate discharge planning including home exercise education. Supervise other personnel where applicable and complete OASIS assessments per state regulations. Work Monday through Friday full-time across Clarke, Frederick, and Warren counties.
