Home Health Occupational Therapist OT Full Time

Assess, teach, and deliver occupational therapy interventions directed by the physician to optimize patient well-being. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Provide written evaluations, plans of care, and discharge education while maintaining accurate OASIS documentation to ensure compliance and avoid denials. Collaborate with interdisciplinary team members, physicians, and caregivers to coordinate services and modify treatment goals as needed. Supervise other personnel where applicable. Full-time Monday through Friday visits across Clarke, Frederick, and Warren counties with competitive pay and comprehensive benefits.
