Home Health Occupational Therapist OT Full Time

Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations and discharge care plans, collaborating with interdisciplinary team members, physicians, and families to optimize patient well-being. Maintain documentation to ensure compliance with physician orders and payer requirements, and supervise other personnel as applicable. Full-time Monday through Friday visits in Lee County.
