Home Health Occupational Therapist OT Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels for patients in Clarke, Frederick, and Warren counties. Administer, evaluate, and interpret tests to determine functional goals while delivering occupational therapy services in compliance with physician orders. Provide written evaluations, modify plans of care, and manage discharge planning including education on home exercise regimens. Collaborate with interdisciplinary team members, physicians, and caregivers to optimize patient well-being and coordinate services. Maintain accurate OASIS documentation and ensure compliance with payer requirements to prevent clinical denials. Supervise other personnel as applicable and serve as a clinical resource consultant for staff.
