Home Health Occupational Therapist OT Full Time

Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function, while administering tests to determine functional goals and outcomes. Provide written evaluations defining needs and progress, maintain plans through timely documentation, and modify goals or discharge care as appropriate. Collaborate with interdisciplinary team members, physicians, and families to optimize patient well-being and coordinate services. Supervise other personnel as applicable and complete OASIS assessments where permitted. Full-time Monday through Friday coverage in Dekalb and South Gwinnett counties.
