Job Summary
Assess, teach, and deliver occupational therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify plans as needed, and document client performance to ensure compliance with physician orders and payer requirements. Collaborate with interdisciplinary team members, nurses, and families to optimize patient well-being, manage discharge planning, and supervise other personnel. Complete OASIS assessments where applicable and maintain effective communication across the healthcare team.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
- Valid Drivers License
