Home Health Occupational Therapist OT Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels for patients in Dekalb and South Gwinnett counties. Administer, evaluate, and interpret tests to determine functional goals, then provide written evaluations and plans of care defining needs, progress, and expected outcomes. Deliver occupational therapy services per physician orders while maintaining plans through timely documentation, goal modification, and discharge care planning. Collaborate with interdisciplinary team members, physicians, and families to optimize patient well-being and coordinate services. Supervise other personnel as applicable and complete OASIS assessments where permitted.
