Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to determine functional goals and outcomes, then provide written evaluations defining occupational therapy needs, progress, and expected outcomes. Deliver therapy services compliant with physician orders while maintaining plans of care through timely documentation and modifying goals as needed. Collaborate with interdisciplinary team members, physicians, nurses, and caregivers to optimize patient well-being, coordinate discharge planning, and educate families on home exercise plans. Supervise other personnel where applicable and complete OASIS assessments. Perform duties with minimal supervision, ensuring documentation accuracy to prevent clinical denials.
Required Qualifications
- Degree in Occupational Therapy from an accredited university/program
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Valid CPR
